This form is required only for rental situations (after following the 3-year rule), not for new purchases.
You must also include a copy of the lease or your application will be denied.

"The Owner must occupy/own unit for no less than three years prior
to any new rental lease being approved."
Condo documents and bylaws are available for purchase by prospective buyers for $75. Please contact [email protected] if you need such documents.
Requests for Estoppel/Questionnaire certificates carry a fee of $125. Regular mail or drop off through our office door mail slot only. We cannot accept FedEx, etc.
As a reminder, ANY person staying in a unit on the property for more than two weeks in a year is considered a resident and must be screened according to the bylaws of the Condo Association.
$100 Application fee per person over 18 years of age. Make checks payable to Middle River Homes.
